INSTRUCTIONS FOR THE AUTHOR
Reporting Standards
The author must present the results of accurate research conducted accompanied by an objective and significant discussion. The basic data should be accurately displayed in the article. The article should include enough information and references that others can refer to in future research. Intentional fraud on reports is considered unethical and unacceptable behavior.
Data Access and Storage
The author is required to provide raw data relating to the article for editorial review, and must be prepared to provide public access to such data whenever possible (in accordance with the ALPSP-STM Data Statement and Database requirements), and in any case must be prepared to store such data. for some time after publication.
Originality and Plagiarism
The author must ensure that the submitted article is entirely original, and should be cited or cited appropriately if the author has used other people's research and/or words.
Dual, Recurring, and Concurrent Publishing
Authors should generally not publish manuscripts with the same research in more than one journal or other than major publications. Submitting the same manuscript in more than one journal simultaneously is unethical and unacceptable behavior.
Source Recognition
Recognition of the work of others must be carried out. The author must cite all the literature used in the article.
Article Ownership
Ownership should be limited to those who have made a significant contribution to the design, design, implementation or interpretation of the results of the proposed research. Everyone who has contributed must be listed as a writer. If there are others who have participated in a particular substantive aspect of the article writing or research process, then they must be recognized or listed as contributors. The lead author should ensure a list of all contributing authors, and all contributing authors have viewed and approved the final article to be submitted for publication.
Danger and Subject of Man or Animal
If the research involves chemicals, procedures or equipment that are hazardous and inherent in their use, the author must clearly identify these in the manuscript of the article.
Disclosure and Conflict of Interest
All authors must describe in the manuscript any financial or substantive conflicts of interest that may affect the outcome or interpretation of the article. All sources of financial support for research must also be disclosed.
Fundamental Mistakes in Published Articles
When the author finds errors or inaccuracies in the manuscript of the published article, it is the author's obligation to immediately notify the editor or publisher of the journal and work with the editor to correct it or withdraw it.
AUTHOR'S GUIDELINES
Articles submitted to the (JUK-Medifa) of the Jurnal Kesehatan, Rekam Medis dan Farmasi must follow the writing procedure (template link) as follows:

The manuscript of the article is written in Indonesian and the submission is carried out online through the website (JUK-Medifa) of the Jurnal Kesehatan, Rekam Medis dan Farmasi. Submission of article manuscripts is free of charge but publication is charged. The author must register in advance.

The submitted article is the result of research with a maximum number of 15 pages. A4 format paper size (21x 29.7 cm); with margins: bottom 2.54 cm, top 2.54 cm, left 2.54 cm, and right 2.54 cm. Header and Footer = 1.27 cm.
- Titles, Abstracts, and keywords are written in English.

The title of the manuscript is no more than 15 words, provided that the format of the title case, the hat is large, middle, bold, using the Type Book Antiqua, size 14, space 1.

The abstract does not contain images or tables, in a justified format, using Timew New Roman type size 9, space 1, not exceeding 250 words.
- Keywords consist of 3-5 words/phrases, separated by commas, in a justified format, using the font Book Antiqua size 9, space 1.

Corresponding Author consists of correspondent name, Affiliate, Affiliate Address and Correspondent Email using Book Antiqua font size 9, space 1.

- Content of the Article Manuscript

The main text of the article should be written in 1-column format with a column spacing of 1 cm, justified, using the Book Antiqua type, size 11, indentation of the first line 5 mm, 1 space.

Consists of: Introduction, Research Methods, Results and Discussions, Conclusions, followed by Recognition and Reference.

Introduction: reveals the background of research problems, related studies that already exist, differences from previous research so as to give rise to the novelty of research, problem formulation/identification, writing objectives, expectations to be achieved from writing, and scientific benefits of writing.

Research methods: describe the data, determination of populations and samples, analysis tools used, and if any can include a research Fishbone Diagram.

Results and Discussions: reveal research findings based on field data obtained by questionnaires, surveys, documents, interviews, observations and other data collection techniques. The results of the study are analyzed by interpreting with certain techniques and synthesizing with certain theories (it can also come from the thoughts of the author). The results of the study should be concise and clear. The discussion should analyze the significance of the results of the study, avoiding repetition. Avoid discussing excerpts and literature that are too broad in this section.

Conclusion: contains a summary of the answers to the purpose of writing, the possibility of implementing or developing research and suggestions or recommendations for further research.

- Heading can only be created up to 3 levels.
- Heading 1: uppercase case, uppercase, left-aligned, bold, using the font Book Antiqua, size 1 1,1 space, using Roman numbering with periods.
- Heading 2: title case, left-aligned, bold, using Book Antiqua font, size 11, 1 space, using capital letter numbering followed by periods.
- Heading 3: title case, left-aligned, italics, using the Book Antiqua font, size 11, 1 space, using numbering with numbers followed by closing parentheses.
- Heading 4 is not accepted in article writing.

Figures and tables should use black and white, if they are made in other colors, it must be ensured that numbers and tables can be read clearly when printed in black and white. Images and tables should be clearly legible and made in a size proportional to the entire page. Image headings and table headings should be numbered, with Book Antiqua font size 11, space 1. To make it easier to customize the appearance of the article/layout, please include the images or tables in .tif/.jpg/.jpeg format in a separate folder.

Mathematical equations are clearly written, use a sequence of numbering, and are supplemented with the necessary information.
- Manuscripts use page numbers including headers and footers.

All hypertext links and section bookmarks will be removed from the article script. If your article refers to an email address or URL, you'll need to type in the full email address or URL.
- Citations and references are written using Mendeley's model.

References / Bibliography written in the font Book Antiqua Size 11, 1 space, 0 pt. The reference writing format follows the IEEE Style 6th Edition as below.
- Primary references must be at least 80% of at least 10 references.
- References should use the latest sources for at least the last 10 years.
- In detail, the procedure for writing an article script can be downloaded on the template that has been provided.

The editorial board has the authority to reject the manuscript of the article based on the reasons stated by the peer reviewer and make necessary changes or adjustments regarding the use of language without changing the substance of the article manuscript. The editing of the content of the manuscript will be consulted with the author first.
- Manuscript Preparation and Style
The author must fill out an agreement when submitting his article to the journal, such as:

An Ethics Statement containing a statement that the submitted article has never been published or is in the process of being reviewed in another journal,

Copyright Transfer Agreement (CTA link) containing a statement that the author submits his copyright to the journal and the article has followed the journal writing format, and other information

Authorship Agreement which contains a statement that the author agrees with the order of the author's name in the published manuscript.
This approval will appear at the beginning when the author will submit his article. The author will be asked to upload three approvals in the additional file section and tick off all the necessary information as a form of the author's approval with the rules applied by the journal manager.
Submission Preparation Checklist
As part of the submission process, authors are required to check their submissions for compliance with all of the following items, and submissions may be returned to authors who do not comply with these guidelines.
- The submitted manuscript has never been published in any journal
- Script files available in Microsoft Word document format
- If available, please include a reference URL.
- The text is spaced 1, the font Book Antiqua size 11, all images, tables are placed in the text according to the explanation.

- This journal uses Double Blind Review
